Background
Pentoxifylline is a drug commonly sold by Aventis under the brand name Trental. Its chemical name is 1-(5-oxohexyl)-3, 7-dimethylxanthine. Pentoxifylline is a xanthine derivative. Xanthines help with bronchoconstriction, but hinder blood flow.
Other brand names include Pentox, Pentoxil, and Flexital. “Like other methylated xanthine derivatives, pentoxifylline is a competitive nonselective phosphodiesterase inhibitor which raises intracellular cAMP, activates PKA, inhibits TNF and leukotriene synthesis, and reduces inflammation and innate immunity”. In effect it is used to treat asthma and allergic reactions being a bronchodilator.
Pentoxifylline is also an antagonist at adenosine 2 receptors. By acting as an antagonist, it blocks blood flow not helps it. The A2A receptor regulates myocardial blood flow by vasodilating the coronary arteries, which increases blood flow to the myocardium, but may lead to hypotension. If this drug is used it could do the opposite.
What however is extraordinary is that, according to Wikipedia [and of course they may be wrong, but if they are, this is dangerous information to put on the web], this drug is used to
- treat ' intermittent claudication resulting from obstructed arteries in the limbs and vascular dementia'
- treat some sorts of brain damage
- help prevent stroke
- manage sickle cell disease.
- treat nausea and headaches in the mountains (altitude sickness),
This may explain why so may people have died taking it –
On Jan, 17, 20172,895 people reported to have side effects when taking Pentoxifylline. Among them, 79 people (2.73%) have Death
If the above is true this is mis prescription on a vast scale. This paper describes its correct use
“PTX exhibits its anti-inflammatory effects in LPS-induced lung injury through an A2A-dependent pathway. These results will help to better understand previous conflicting data on PTX in inflammation and will direct further studies to consider the predominant role of A2A – PMID: 23699177 “
