Theophylline , shortened to Theophyl , also known as dimethylxanthine, trade name Theo-dur is a methylxanthine drug used in therapy for respiratory diseases such as COPD [chronic obstructive pulmonary disease] and asthma under a variety of brand names. Because of its numerous side-effects, the drug is now rarely administered for clinical use. It is a member of the xanthine family and bears structural and pharmacological similarity to caffeine. It is naturally found in tea, although in trace amounts. It is also found in cocoa beans. Amounts as high as 3.7 mg/g have been reported in Criollo cocoa beans. Theophylline is both a
- competitive nonselective phosphodiesterase inhibitor which raises intracellular cAMP, activates PKA, inhibits TNF-alpha and inhibits leukotriene synthesis
- nonselective adenosine receptor antagonist, antagonizing A1, A2, and A3 receptors almost equally
Its effects are thus highly complex because it affects the heart via the adenoside receptors but also has anti-inflammatory effects:
- relaxing bronchial smooth muscle
- increasing heart muscle contractility and efficiency
- increasing heart rate:
- increasing blood pressure
- increasing renal blood flow
- some anti-inflammatory effects
- central nervous system stimulatory effect mainly on the medullary respiratory center.
At first sight, the stimulatory effects ought to outweigh the effects of the anti-inflammatory effects, but in fact they tend to exacerbate the effects adding to the light headeness and lack of oxygen at too high a dose. Theophylline can reach toxic levels when taken with fatty meals, so overdose can be accidental
