If you have to self help and can’t press the perineum with your heel because you are simply not flexible enough, a tennis ball on the perineum works almost as well. 

The reason you use the heel is that you can adjust its positioning until you think you have got it ‘right’ and do this whilst maintaining a relaxed posture, if you simply bung a tennis ball under you, you have to break the posture to keep on adjusting it and it does have a tendency to pop out!!
