Lorna Middleton foretold the death of a Russian cosmonaut.  In April 1967, she had a vision:

"someone is going to the moon. He's terrified. He's burning."

The following day cosmonaut Vladimir Komarov died in the Russian spaceship Soyuz I. After the ship had orbited the earth for twenty-four hours, the re-entry parachute (cosmonauts return on land) became tangled and the ship crashed.

Whether Komarov was "burning" is uncertain, but there is evidence that something was wrong during the orbits, when there were no reports from Tass, the Soviet news agency. Also, Komarov was not wearing a spacesuit and was vulnerable to a change of conditions within the space-craft.
